Over 1,200 people with disabilities are able to live in their homes, outside of nursing homes with assistance from personal care workers in our program.  We are a WI Medicaid Certified Provider - prior approval is needed before services start.

Attendant Referrals

We help with recruitment and referral for people with disabilities who want to hire their own personal care attendant, providing training for the individual with a disability and for personal care attendants. Participants in the program are paid by the individual with a disability, their direct employer.

Nursing Home Transitions

 

We educate people with disabilities about their choices for community-based living. We teach people with disabilities decision-making skills, how to be their own advocates as they deal with all of the people involved in a transition, the skills necessary for them to live in the community, and help them identify community resources to assist them in transitioning back to their own home or apartment.

WI AODA Project

A state-wide project for people who are Deaf, Deaf-Blind or Hard of Hearing and have AODA  (alchohol or other drug abuse) issues.  This program works to increase access to culturally-sensitive, language-appropriate and accessible AODA treatment/recovery services for this population. We work to reduce communication, language, cultural and other barriers to services for consumer participation.  We do NOT provide one-on-one counseling.  We assist with information/referral to existing providers and provide information to AODA service providers on how to become more accessible.

Medicaid Purchase Plan (MAPP) and Health & Employment Counseling (HEC)

Wisconsin's Medicaid Purchase Plan (MAPP) offers people with disabilities who are working or interested in working the opportunity to buy health care coverage through the Wisconsin Medicaid Program. Depending on an individual's income, a premium payment may be required for this health care coverage.

 

To qualify for the Medicaid Purchase Plan, the following requirements must be met:

  • Be at least 18 years old and reside in Wisconsin.
  • Have individual countable assets of less than $15,000. (Countable assets include savings, life insurance policies, stocks or bonds. A home or one vehicle is not considered a countable asset).
  • Be determined to have a disability.
  • Have an adjusted family income of less than 250% of the federal poverty level, based on your family size.
  • Be employed or be enrolled in the Health & Employment Counseling (HEC) Program
