For those 6 months - 17 years old, the presence of a parent or legal guardian is required for consent.
Please remember to bring your indentification and proof of insurance, if available.
Your insurance will be billed, and you will not receive a separate bill.
If you do not have insurance coverage, we will discuss options during your appointment.
All eligible individuals will be vaccinated regardless of ID, documentation or insurance status.
